Private Pay Rate

For clients choosing or needing to privately pay (fee-for-service) for their sessions, my rate is $140 for a 60-minute session and $180 for initial assessments/assessments as more time is involved.  This fee is due at the time service is provided.

Insurance Coverage

I can work with clients with Blue Cross/Blue Shield insurance, Wellpoint Iowa (Medicaid), and Iowa Total Care (Medicaid) in addition to clients choosing to pay for therapy out-of-pocket.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

At this time, I do not work with clients with Health Partners, Medicare, Molina Healthcare of Iowa, Cedar Valley Community Health Care, Midland’s Choice, UnitedHealthcare insurance or Aetna insurance coverage.

Payment

Cash, check, credit card, and health savings account payments are accepted by State Street Counseling Services.

Running Late | Cancellation | No Show | Policies & Fees

I dedicate your reserved appointment time exclusively for you.

If you need to cancel or reschedule your appointment, please do so at least 48 business hours in advance of your scheduled appointment time—this does not include holidays and weekends. For example, if you need to reschedule your 3 PM Thursday appointment, please do so by 3 PM on Tuesday and if you were to need to cancel your 9 AM Monday appointment, do so by 9 AM on Thursday.

Exceptions are considered for sudden illness and emergencies but generally speaking:

• If less than 48 business hours’ notice is given, you will be charged a $75 late fee.
• If you fail to inform me at all and do not show up for your appointment, you will be charged the full session amount of $175 ($140 for cash-pay clients).
• If you are running late and arrive 20 minutes or more past your appointment time, this will be considered a late cancellation and you will be assessed the $75 late fee and asked to reschedule.

Please note these late charges need to be paid in full prior to rescheduling and are not covered by insurance.  If no attempts are made to pay these fees, your credit card on file will be charged and services will be terminated.

Three late cancellations or no-shows within a 4- month period will result in termination of services.
